***We no longer accept UHC/OPTUM plans for out of network billing, please ask if you have questions about superbills to your insurance for reimbursement.

Note: Insurance covers individual and family therapy (not couple's or relationship counseling) and there must be a diagnosis for insurance to cover services. There is usually a co-pay and possibly a yearly deductible. You can also use FSA/ HSA cards! 

 

Connective Therapy Collective (CTC) requires that all clients add a card on file for any incurred
expenses for services. CTC will run your card at the end of each attended session for any out of
pocket fees set by CTC for self pay options or as is determined by your insurance (including copays,
coinsurance, or deductibles).

CANCELATION POLICY

CTC requires 48 hours to cancel or reschedule your appointment with your provider.

Any cancellations within 48 hours or no shows to scheduled appointments are subject to a fee of 50% of their session fee if they are utilizing self pay options, or a fee of $75 when billing commercial insurance.

MEDICAID/OHP NOTE:
Medicaid/OHP clients are not subject to late cancellation fees, however, if there are more than 3 late cancellations or no shows within a 6 month period, you may be subject to losing your time slot and being put back on the waitlist for one of our providers in order to limit waitlist wait times.
